Dr. Sebastian Sepulveda, MD, is a physician in Chelmsford, Massachusetts. He has nearly 30 years’ experience as a doctor and former associate professor of medicine. He is a full-time hospitalist, specializing in internal medicine and nephrology, and works with end of life care patients at Lowell General Hospital. He is the author of the book, At Death’s Door: End of Life Stories From The Bedside which was made into a TV series called Death's Door. Dr. Sepulveda has recently been working on a documentary on the same topic.

- I think without mindfulness, I couldn't do the job. I think you gotta know who you are, know what you are doing and keep in touch with reality. I think those things are part of mindfulness.
- If there is a chance to transfer some of that mindfulness to the patients and relatives, it will be precious and I try to do that too.

- I don't focus on breathing, not at all. For some reason, I don't need relaxation techniques or anything like that. I just work and it works for me. Right. My breathing is just on automatic pilot.

Effect on Emotions
- When I was younger I definitely had a lot of anger issues and I think I attributed it to the frustration of being bullied and going through the frustration of wanting my mind to be working like everyone else's. And it wasn't.
- And so I was constantly in a state of frustration and anger. And, I think what mindfulness and meditation has done for me is it's helped me to realize that everyone's not the same and that we all have our strengths in certain areas. And to be okay with that, to let go of needing to be like everyone else and to become more me.
- You are the only you and that's your true power.
Thoughts on Breathing
- I would say breath is probably the biggest for me in the meditation routine because I feel like 10 slow inhales and exhales can change anyone's mood, just in the snap of a finger. And I think that it gives more oxygen to your body, your organs work different, your whole system functions differently when you give it the oxygen that it needs.
- We go through life in such a hustle-bustle that we forget that we're not even breathing at all. We're just kind of surviving there. And so breath is very, very powerful for altering moods and setting the tone for my meditation. It's very powerful.

Bullying Story
- Yeah, so right off the bat, the first thing I think about is having dyslexia. I was made fun of a tremendous amount in school. In dealing with that learning disability. I took longer for tests and I had a hard time in school when I was younger.
- The big thing was that you would read chapters and then copy those chapters to page and they wanted you to learn from that. And I was just not that kind of person. Plus I had a hard time from that transference from the book to the page. But if someone would talk about it in front of me, I would remember every single thing that they said.
- I was very into the auditory learning. You know, basically sound and voice. Still to this day, if an actor comes on screen and if I don't see them, I can just recognize them from their voice even if it's an obscure person that was on a show from six months ago that was just like a guest appearance. That was just always the way I learned.
- I was called dumb and stupid and just had a really hard time. Fourth grade through eighth grade, it was just really hard. I got bullied a lot and had a lot of issues where I didn't want to go to school.

- It's made me a more calm, level, and relaxed person. Understanding that I can't be perfect nor will I ever be perfect, what I do is, I simply try to be the best version of me. I have my bad moments, but I catch myself. I'm more self-aware now.
Thoughts on Breathing
- Breathing is the key to mindfulness. If you are holding your breath you are turning on the fight or flight.
- You are most likely either chest breathing or you are stimulating what is called the sympathetic nervous system. It's hard to be relaxed, it's hard to produce gaba, serotonin, all the relaxation neurotransmitters if you are holding your breath. Most people do. I used to. Or chest breathing under stress.
- What you do is you go back to belly breathing. You can look up diaphragmatic breathing or belly breathing. I did an interview on this with one of the foremost doctors on breathing. You want to breathe in through the nasal passages and if you have clogged nasal passages, then you need to work on why they're clogged and inflamed like mine were.
- You're going to focus back on the one thing you really can control, and that's your breath. By going back to your breath it will take your mind off the situation and then you can refocus back on that situation.

- Sometimes we can really get drawn into someone else's bad mood or negativity or drama or issue, but when you're being much more mindful about your life you're much more inclined to be drawn into their issues and struggles as well.

- When I meditate in the morning, I really am connecting with my breath. That helps me connect with my being.
- Whenever I find myself completely frazzled and stressed, I just come back to breathing, three deep breaths, breathing in, 1,2,3, and out, 1,2,3. It just centers you so you can get back to that place of present.

- I would say it affected my ability to respectfully call myself out, if I'm in that stressful mode. To recognize that even that stressful mode, anger, resentment, anxiety, whatever it may be, is a moment that from my language is already hypnotic in nature.
- Let's use a simple definition of a word that some people would have connotations of, the word trance.
- That's a loaded word, but my definition is rather simple: When internal perception carries more meaning than external reality.
- So that mindful moment just to take a deep breath, observe the environment that I'm actually in, become aware of my body occupying space is often that number one strategy to solve whatever stress or frustration may be there.
Thoughts on Breathing
- You always get to bring your breath with you, and as long as you're breathing, you're going with the old George Burns line that I woke up in the morning, I read the obituaries, I didn't see my name, it was going to be a good day.
- But it has that ability to centralize the focus of the breathing and even at times to play with breathing that, from an almost hypnotic standpoint, to play with taking that breath and moving that in to a part of the body just dissolves away whatever stress or unresourceful emotion would be there.

- Let me put it this way. I was always so afraid of feeling myself because I was afraid of what I would feel. And it wasn't until I got mindful, till I got present with my emotions that actually my life changed.
- So mindfulness has played a huge role in every aspect of my life because of the fact that I was willing to slow down and feel myself.
Thoughts on Breathing
- Breathing, when I'm spinning, I can get really woo and spin and spray. So that's the key tool to coming back. I love taking in a very mindful breath of breathing in four seconds, holding it at the top and then breathing out four seconds and then holding it at the bottom.
- And doing that just a couple of times that really centers me and it just, it, it makes me just take all my energy intention out of the world that I'm in or what I'm thinking about that I'm spinning about and just coming back to the moment and really grounding me. I do that a lot.

Bullying Story
- I don't get bullied and I do not bully because I understand that the whole bullying situation is based on law of attraction. You have to have a victim and you have to have a bully and you have to have someone being willing to be bullied, whether they're aware of it or not. Because what's happening with the bully is that somewhere in their life they're being bullied and they're trying to find their power.
- And what's been modeled for them; what is a source for them because that's what they've seen is that they then bully. And so I have a situation, I think it's been about four years ago now.
- My son Maxim who is now eight, but when he was in kindergarten just going into kindergarten, there was a little kid named Gibson and Gibson would one day pretend to be his friend and then the next day would push them down and hurt him. I would get so angry. [I would say to Maxim], why aren't you speaking up? Why aren't you telling somebody? And he was just like, because I want him to be my friend.
- And it's like, wake up Maxim, he can't bully you if you say no, if you stand up to him. The only way bullying can continue is if a person continues to allow it and doesn't do something about it. When you show a bully your power and you're saying no, not to me, the bully will go somewhere else or they'll try to find it somewhere else, but they're not going to bully you.
- And we had to get in that with him in many, many conversations. And it was really interesting. He just finally woke up, I think it was about six years old and this kid was kicked out of the school and he went for his blue belt in karate. At the age of six and something just snapped in him. And he, he started with his brother, um, at like these little amusement park places.
- There was a bully that was bullying other little kids and he walked up to the bully who was bigger than him and he said, hey, go pick on someone your own size, you know, standing up for this kid. He really grew in his power and now, if anybody was to ever to bully either one of my kids, they'd be like, what are you doing here?
- Because they really own that, you're only going to bully me because you're trying to feel better about yourself in some way. And the only way that they know how to do that is because they need a power over somebody and you're not going to power over me. So they, they stand up for themselves.

- It's been a wonderful thing. It has enriched my life a lot, I think mainly because I feel a lot more connection to the surroundings, and in daily life, I try to spend time pausing for beauty and looking at the beauty of the natural world.
- I'm very fortunate, Tucson is a lovely town, and we've got the Catalina mountains and the Santa Ritas, so we're surrounded by mountains, the clouds, the sun. It's just a very beautiful place and I take enough time to really pause and take that in, and that to me really is the core of mindfulness, being able to appreciate your daily life.
Thoughts on Breathing
- Breathing is an Absolutely crucial [part of my mindfulness].. Whenever you feel a little bit tense or a little bit upset or any kind of stress at all, just do a quick little minute or two exercise where you count your breaths. That's all.
- So for me, whenever I begin, it always begins with, 'Take a deep breath. Hold it for three seconds, release it for three seconds' and you do that pattern.
- And the next thing you know, everything becomes manageable again and you're focused and you're in the moment. That breathing exercise helps calm the mind down and that is a crucial part of any meditation exercise.

Bullying Story
- There's a lot of conflict in prisons and violence and conflict are kind of at the core of that experience because you get a lot of people that really are rule breakers and you get them all together and stuff them in one room and they're not allowed to leave. That's prison.
- So there was this one guy, and this was in Texas, in a prison in Texas, who just couldn't get along with anybody in his cartel member. And he would, he would bully because he was a very large guy and worked out a lot.
- Making a long story short, he was picking on one guy who is kind of small and thin, a native American kid. It was on the weight pile and there was a number of us that were around that really liked this kid. He was a good kid, from one of the tribes there in Texas. And we just said, "You know what, we're not putting up with this anymore."
- And we surrounded this guy in the middle of this session where he's belittling this kid and really, really overbearing. And we surrounded him and were just like, "Look at yourself. Look at who you are. You're 280 pounds of solid meat. You bench press 500 pounds. And here's this kid who's 100 pounds and you're bullying this kid. What are you doing? Look at yourself. Who are you?"
- And the shame of the group, the ability of the group to just focus their energy and saying, "You know what, you are so close to being an outcast, we are done with you. We are so sick of this." That had an enormous impact.
- He went quiet, he gathered up his things, he left and he was a lot more reflective and a lot more thoughtful and that, of everybody just dirty looking him, giving him a hard time, and saying this is not acceptable behavior, and that bringing the present moment, that mindfulness moment, if you will.
- Interfering there had a real impact and it had an impact on other people that had that kind of personality as well, because they saw what happened that, and this is the essence of society, is that if you have a group of people that share the same kind of values and vision, that they really can make an impact, really can have an impact on your surrounding.
- Even in a society that's violent and sort of out of control like a prison. When you mentioned bullying, I thought immediately of that little get together where we were like, "No, no, no, we're done. This is enough."

Effect on Emotions
- Mindfulness, as I mentioned at the beginning, to me is a muscle and it's something that was very confusing for me at first.
- It took a lot of time, but now mindfulness has completely changed my life because I don't have to cultivate it as much.
- The garden is already there and so when I used to have to be very present and kind of reengage with the present moment, I feel like engaging with being in the present moment and truly seeing now is more of my default instead of it being like five percent of my day where I have to be very intentional about it.
- I've entered probably just in the last six months that shift where now more than 50 percent of my day, I'm truly awake. That's been a mixture of having the time and space with which to cultivate that.
Thoughts on Breathing
- This is something when I was really stressed and I was working all the time, I had to kind of, come into meditation and focus on my breath once I got in like 20 minutes a day, once I got out of the corporate world and was truly financially independent that I could wake up and do anything that I wanted with my day, my breath naturally settled. So now I actually don't have to.
- Unless I'm really stressed out, I don't have to come home to my breath. My breath is my default home. My blood pressure dropped.
- It's kind of like the length of your breath is the length of your life, and I really feel like the length of my breath is much longer simply because it took me like nine months to detox from leaving the corporate world.

Bullying Story
- So there was a neighbor, there was a guy that lived on the street behind me and I'd known him growing up. He was one of three siblings and he was just one of these people who now in hindsight I realized was quite lonely and quite sad.
- He always just reeked of body odor, just like intense. I think it was a mixture of his parental situation, I don't think his parents were very present. It was the environment he grew up in.
- I would make comments around his body odor and, that he was just a jerk.
- This wasn't a very long period of time, but it was all facilitated by some friends that I was hanging out with. I remember that he got upset about it.
- He told his mom, his mom called my mom and my mom being like, you know, do you want this to be your character? And all those sorts of things.
- You know, I felt really bad about it. I remember going over and apologizing earnestly, but then that was, I was actually a bully in that sense before I was bullied myself.
- And then two years later in eighth grade, there was a kid who would just always bully me really intensely.
